REQUESTS MAY BE MADE IN PERSON AT CITY HALL OR BY MAIL

To Request Of A Certified Copy of a Birth Certificate By Mail

  • Full name of child, including first, middle and last name at time of birth
  • Date of birth
  • Child's Mother's first, middle and maiden name
  • Child's Father's first, middle and last name
  • Relationship to the name of the request
  • Genealogist ID # (if applicable)
  • Name and mailing address where we should send the certificate(s)
  • Phone number
  • Number of certified copies you are requesting
  • Original Signature on the request

Copy of Driver's License, Passport or Goverment ID or two copies of utility bills, bank statements, vehicle registration, personal check w/address, previously issued vital record, social security card, DD214, pay stub, w-2, voter registration card.

  • Full name of the deceased, including first, middle and last name at time of death
  • Date of death
  • Your relationship to the deceased
  • Genealogist ID # (if applicable)
  • Name and mailing address where we should send the certificate(s)
  • Phone number
  • Number of certified copies you are requesting
  • Original signature on the request

The fees are $15.00 for the first copy and $6.00 for each additional copy.
